All forms listed below are required and must be submitted to Ware prior to the first day of school.
Health and Immunization
NEW STUDENTS
Virginia State law requires documentary proof of immunizations of all students prior to admission by a school. Admission to Ware Academy can not be considered complete until a copy of current immunization records has been submitted.
ALL STUDENTS
Documentary proof of adequate age-appropriate immunizations with the prescribed number of doses of vaccine must be provided periodically to the school. Updated immunization records must be provided to Ware Academy for all students entering Kindergarten and Sixth Grade. These records will then be reported to the State.
In addition, students that participate in Junior Varsity or Varsity sports are required to provide the school with proof of a physical examination each year. Sports physicals must be completed no earlier than June of each year in order to cover the school sports period (i.e., a physical in the month of June will cover the period through the following May).
Emergency Information and Permissions
The following forms allow the school to contact appropriate individuals during both emergency and important non-emergency situations, or grant specific permissions as explained below. All forms listed below can be downloaded from the school’s website under the Current News tab on the From Main Office page.
This form is required by the State and must be on file for all students in case of an emergency
(whether participating in the AfterCare Program or not).

Emergency Medical Information Form (PDF)
A signed copy of this form must be submitted for each child attending Ware Academy.
The Code of Virginia requires schools to report the presence or suspected presence of any
epidemic or outbreak situation.
Provides detailed contact information for parents and/or other designated persons to be contacted
in case of both emergency and non-emergency situations that may arise.
This form provides permission for persons other than parents to take a child from the school.
Please note that if someone is not listed on this form, the child will not be released to them without a
authorizing note or phone call to the office. Also note that no student will be allowed to participate in
any off-campus field trips, sports activities, etc. without specifically granting permission on this form.
On occasion, students may be photographed and those photos used in school publications or for
marketing. Enrollment at Ware implies permission to publish photographs or interviews. The
Photo Release Form confirms that permission or notes any exceptions.
This form provides contact information so grandparents may be notified of events or programs in
which their grandchild will participate or those of interest to the school community. Ware families
may elect to limit information sent to grandparents.
