a.Registration Form [Word Template] This form can be completed online; then saved and printed for submission to Ware. You can also email it to SC Registration Desk.

b.  Registration Form (PDF file)This form can be printed; then completed handwritten and submitted to Ware.

II. Please print and sign the following agreement:

a. Summer Camp Agreement

In order to reserve a spot in the camp(s) of your choice, Ware Academy must receive full payment, completed registration form and signed agreement.
